2026-2027 Competitive Season · Parent/Guardian Consent

Travel, Transportation & Overnight Lodging Authorization

Complete after roster acceptance and before any club-arranged transportation or overnight travel. Each permission below is independent. Declining one does not automatically prevent tournament participation if the family makes approved alternative arrangements.

1Athlete & Parent/Guardian Information
2Transportation Permission

I authorize only the selections marked below for transportation to approved Venom Volleyball Club practices, training, tournaments, and club events during this season. Drivers must be authorized by the club, appropriately licensed, and meet applicable insurance and youth-protection requirements.

Transport in a coach’s or volunteer’s personal vehicle is not authorized by this form unless separately approved by the club and parent/guardian in compliance with applicable policies. Venom will communicate pickup/drop-off plans in advance. Seat belts are required; unauthorized detours and one-on-one adult/minor transportation are prohibited except as allowed by applicable safeguards and written consents.

3Overnight Travel & Team Hotel Lodging

For overnight events, Venom will provide the destination, dates, hotel name/address, transportation plan, supervision contacts, and relevant trip rules before departure. Season-long permission below does not replace event-specific acknowledgment or any required safeguarding consent.

Hotel accommodation selection (choose one):

Athletes may be assigned to shared rooms with other athletes. Adults will not share sleeping accommodations with minor athletes except as expressly permitted under applicable youth-protection rules. Venom will implement supervision, curfew, room-access, and check-in procedures consistent with governing-body requirements. No promise of continuous individual supervision is made.

4Conduct & Family Responsibilities
  • Athletes must follow curfews, room assignments, travel instructions, and team conduct policies.
  • Athletes may not leave the team group, hotel, or assigned room without approved permission.
  • Guardians must provide accurate emergency information, disclose relevant needs, and promptly report changes.
  • Families must review each overnight itinerary and notify the club if consent or arrangements change.
5Emergency Medical Care

If a parent/guardian cannot be reached promptly, I authorize designated Venom representatives to seek emergency evaluation and treatment for my child when reasonably necessary, including contacting emergency medical services. Venom will attempt to contact me as soon as practicable. This is not a general authorization for non-emergency treatment.

6Acknowledgment & Electronic Signature

I affirm I am legally authorized to consent for this athlete; I have read and understand my selections; and I understand I may revoke future permissions in writing, subject to arrangements already underway. This document does not release Venom or others from liability for negligence or unlawful acts. Participation remains subject to applicable USA Volleyball/region safeguarding rules and club policies.
